To receive credit for an approved RateWise™ program, fill in the form below, attach the required documentation, and click the Submit button. Credits are applicable for programs attended in the last 12 months since your program registration.

Required Documentation:  When filling out this form, please have one of the following documents ready: Program Agenda or Certificate of Completion, saved as a PDF file (see "Attachment" section at the bottom of this form).
